Owners should notify EnerVest as soon as possible when their mailing address has changed. For your protection, EnerVest will not accept address changes by telephone. Please submit the applicable form below OR you may submit a request in writing by mail or fax. Please include your name, date of request, owner number, last four digits of your social security number, phone number and signature.
